  • The Importance of Complete Enrollment Forms

    Complete enrollment forms provide the information necessary to ensure smooth, accurate employee enrollment. For example, the form tells us when the participant’s eligibility in the plan should begin, which plan they should be enrolled in, and how much they elect to contribute to the plan. And, timely submission of the enrollment form allows us to keep your group’s eligibility and invoices up to date.

    About Plan Elections 

    Elections must be made prior to the plan-year start date (or the participant effective date). Make sure the form is signed and dated. If no election is indicated, we will assume the employee is choosing to not participate in the plan. 

    Once an election has been made, generally it is irrevocable for the plan year. To change, add, or revoke an election, two conditions must be met: the plan must allow changes and the participant must experience a qualifying event

    Tips for Enrollment Success

    • Review enrollment data for accuracy. Make sure the employee demographics are completed and legible, elections amounts are within the plan minimum and maximum allowed by the plan, and per-pay-period amounts are accurately based on your number of pay periods in the plan year.

    • Health reimbursement arrangement (HRA) enrollment forms typically require dependents.

    • Social Security numbers need to be collected for HRA participants as required by Centers for Medicare & Medicaid Services (CMS).

    • An employer can offer a limited-purpose health flexible spending account (LFSA) for those employees (or spouses) who are actively contributing to an HSA.

    • Send us either enrollment forms or a member census spreadsheet, not both, for your employees.
